    No clarity regarding adding the name of the company on custom form

    I can set up a custom field, but at what point and where should I add the actual name and address?

    What if I have several custom providers? Does each need a new custom field?

    Hi Chris,

    The Custom Company name is added in the add new provider area, from within the Radiology request after clicking on the skeleton icon, but you will likely need a new form per provider and to also list the company on the form by adding free text with the company name into the radiology template and saving that change.

    Having the new provider added allows you to assign the new form you have created to that provider, which will then either have an image of their request form, or just text on  a blank form you populate.

    If you have a specific set out you like but want to add several custom providers, once you have done the first form, duplicate it and rename to the next provider and overwrite the text of the custom provider in that new form, this way you don't have to set up each one from scratch, and can quickly create several templates for your usual local providers but having the other areas (Test ordered, reason, CC's allergies medication etc and the patient details and provider details) maintained across forms.

    I have confirmed that if from the radiology ordering window you click the + sign next to templates for Click to add template, it will populate a blank form with various patient and practitioner details, but if you are not in your primary practice I can see that the doctor provider number is populated with the primary provider number [Preference]ProviderNum but will use the [PracticePreference]FullAddress for the practice, so the latter will always be right but the former only when you are in your primary practice. Beware
