I think the Review correspondence workflow could be improved. I would be interested in how others are managing this. What I think makes things tricky for the Clinicians is that un-reviewed letters can be in 3 places: To be reviewed; printed not reviewed and 3rd Party Unsent, from my perspective if they have not been reviewed they should still be viewable in the To be Reviewed list regardless of whether they are in the other 2 locations. You might print off a referral letter and hand it to the patient during the consultation but on review you may wish to add a cc to the GP or other relevant interested party and also send it to the primary as well, giving it to the patient allows them to have a name and phone number to call to make the appointment. The current setup risks the letters in the other 2 locations just being overlooked and not reviewed, which is why I would like one location for clinicians to look not 3, hence my preference for any unreviewed letter to stay in the To be reviewed list. Any letter with a cc that has secure messaging will automatically go into the 3rd party unsent list.

Also it would be good if flag for follow up did not automatically get ticked if an email is sent could that be an on or off preference?

"Flagged for follow" up in the letter check boxes (automatically ticked when any email sent from a letter), which then translates to  "Flagged, awaiting reply" is unrealistic, most people do not reply!
